Eligibility Requirements
1. Can I use the online ADA enrollment form?
The form is available to retail and individual accountholders.
2. Which types of accounts can I enroll in ADA?
You can enroll the following accounts:
- Savings (individual or joint accounts)
- Build-up Savings Account
- Easy Account
- One Account
- Money Builder Account
- Premium Build Up Savings Account
- Current (individual or joint accounts)
- All Access Account
- All-in-one Access Account (AIOCA)
3. Which accounts cannot be enrolled in ADA or used for an ADA account number change?
- A9 Pensioner Savings
- Deal Payment
- eSecure Savings
- Third Currency Accounts
- Time Deposit
- Treasurer-in-Trust
Enrollment of corporate/non-individual payors is not yet allowed.
The Bank will decline requests bearing the above account types.

5. Can I enroll an SBC account that isn’t in my name?
You must use your own account number for enrollment, but you can make payments for another person’s policy or recurring payment.
For example, if you are an SBC accountholder and you want to pay for the insurance premiums of a relative, indicate the relative’s Policy Name and Policy Number in the Payor Details section.
In the Account Details section, enter the details of the SBC accountholder whose account will be debited for the payments. Make sure the following details match the bank’s records:
- The email address you provide will receive the DocuSign Security Code and other notifications.
- The mobile number you enter will receive the SMS OTP needed to access and sign the document.


6. How do I choose the number of authorized signatories?
The number of signatories you select in the online form depends on your nominated debit account:
- For a single account – Select “1.”
- For a joint account – Select the number of joint accountholders (up to 5).
7. I entered the wrong signatory. Can I still edit it?
No, once the enrollment form is submitted, it cannot be edited. However, you can cancel the enrollment request and submit a new online enrollment form with the correct details.
8. How can I change the signatories for my bank account?
Visit your Security Bank branch to inquire about updating your account signatories. Once the changes are made, you can proceed with the online ADA payor enrollment using the updated details.
Online Enrollment Form
1. How can I access the Online ADA Payor Enrollment Form?
Visit the Security Bank website, hover over Personal, and under Accounts (first column), click Bank Services. Scroll down to the Auto-Debit Arrangement (ADA) Enrollment Form and click Enroll Now.

2. What requests can I make through the online ADA form?
You can submit the following requests:
1. Enrollment
2. Disenrollment
3. Change of SBC debit account no.
3. I submitted the online form but changed my mind. How can I cancel my request before it is processed?
You can cancel your enrollment request in two ways:
Option 1: Self-Service
- Open the email you received with instructions and use the Security Code to access the form.
- In DocuSign, click the arrow next to Finish (upper right corner), then click Decline to Sign.
- In the pop-up window, click Continue, enter your reason for declining, and click Decline to Sign.
- You will receive an email notification confirming that the enrollment was not signed.

Option 2: Request via Security Bank
Contact Security Bank Customer Care and provide your Enrollment Reference Number and Account Name to request cancellation.
You will receive an email notification confirming that the enrollment was not signed.
4. Can I make multiple requests in a day?
Yes, you can submit multiple requests. Each form allows you to make one request type and enroll one debit account. There is no limit on the number of requests you can submit, but each request will be processed separately by the bank.
5. Can I submit multiple request types (Enroll, Disenroll, or Change Account Number) in one form?
No, each form allows only one request type. If you need to make multiple requests, please submit a separate form for each.
6. Can I enroll multiple debit account numbers in one form?
No, you must fill out a separate enrollment form for each debit account.
7. Can I use one form to enroll with multiple ADA merchants?
No, you must fill out a separate enrollment form for each merchant you want to pay.
8. Can I edit my request after submitting it?
No, once the enrollment form is submitted, it cannot be edited. However, you can cancel the request and submit a new form with the correct details.
Docusign
1. How do I sign with DocuSign?
After submitting the online enrollment form, account signatories will receive an email from DocuSign with the subject:
“Action Required: Complete your auto-debit enrollment with DocuSign (Application # _____)”
Follow the instructions in the email to complete the signing process.
2. How long is the SMS OTP valid for?
The SMS OTP is valid for 5 minutes. After 3 invalid attempts, the envelope status will change to “Authentication failed,” and the enrollment form will no longer be available for signature or bank processing. You will need to submit a new enrollment form.
3. Do I need a DocuSign account to sign the enrollment form?
There is no need to create a DocuSign account to sign the application form.
4. Do I need to upload my signature in DocuSign?
No, uploading an image of your signature is optional. You can sign in three ways:
- Upload a signature image
- Select a signature style from the presets
- Draw your signature
Processing and Status Updates
1. How long does it take to process my ADA enrollment?
Your ADA enrollment will be processed within 5 banking days after all account signatories have signed the form using DocuSign.
2. What notifications will I receive?
You will receive the following notifications:
- Email from DocuSign with instructions and a Security Code after submitting the online form
- SMS OTP when accessing the document
- Email reminders if you have not yet signed
- Email confirmation when your enrollment is completed/approved
- Email notification if your enrollment is declined/rejected
- Email notification if the unsigned enrollment form expires and is voided
3. What should I do if I can’t find the Security Code in my Inbox?
If you cannot find the DocuSign email in your Inbox, check the Spam folder.
You may also search your email using the Reference Number shown in the confirmation page after you submitted the online form.
4. What should I do if I haven’t received a status update email after 5 banking days?
First, check your Spam folder or search your mailbox using the Reference Number from the Security Bank email you received after submitting the online enrollment form.
If you still haven’t received an email, send your Application Reference Number or Account Name to [email protected] for assistance.
5. What should I do if I don’t receive the SMS OTP from DocuSign?
Move to an area with better network reception and try to re-generate the OTP. If the issue persists, contact Security Bank Customer Care for assistance.
6. How will I know if my ADA enrollment has been processed?
You will receive an email notification from DocuSign once your ADA enrollment is processed or completed.
7. How will I know if my ADA enrollment has been declined?
You will receive an email notification from DocuSign informing you that your application was declined.
To check the reason:
- Open the email and access the document using the Security Code.
- Click the three dots in the upper right corner next to the Close button.
- Select View Certificate (PDF).
- When the document opens, scroll down or use CTRL+F to search for “Decline Reason”.

8. What happens if you don’t sign the enrollment form in DocuSign?
DocuSign will send you email reminders:
- 3 days after submission of the online form
- Every 7 days thereafter
The bank will not process your enrollment until all account signatories have signed.
If the form remains unsigned, it will expire after 999 days, and you will receive an email stating that the document is voided, meaning the enrollment will no longer be processed.
